the upload image uses mg_themes/images/ice/lang_english/upload.gif... if you are using another theme then you may have to make the image..

the template file have been updated since and included all of those fixes but thanks for doing them anyway..

not sure about this as I have tested the acp side on 3 sites to ceck for blank pages.... 

I will have a look... is anyone else getting this error...?
