This happens when I try to install the MOD level bar 0.0.1b for PHPBB3, probably I failed at the installation but I don't know what's the error, how can I fix it?

The page does not load and the screen is white when I click in 'Nuevo Tema' ('New Topic') or when I want to see a topic :S
