I have installed the MOD as per the help file but having problems with MySQL. Please can you help me installing the MySQL tables

if I try running the http://dive.byethost13.com/phpBB/fap_install.php it doesn't create the tables in my database.

I get the following code when entering the site

I have reloaded the MOD update and am still getting error messages check out http://dive.byethost13.com/phpBB2/
At first I installed phpBB version 3 but after some time realised the install code doesn't correspond with whats in version 3, I then installed version 2.0.2 (I think) and it has the same code which I can edit but still having a problem running as you can see on URL http://dive.byethost13.com/phpBB2/
Any ideas what I'm doing wrong and is what's the difference in version 3 phpBB, can I install the MOD on version 3
HELP PLEASE
