as i see this is on byethost so this can be a problem ...
i think something is blocking your /icy_phoenix.cfg but maybe i m wrong

i know byethost and time ago i tryed and got lot of truble there

did this happent affter install ?

It is a new install or an update?

Try to manually restore the themes table, force all users to style 1 and empty your cache and cookies.
