 Re: Instalation Problems
You need to know your Database Server Hostname (the name of the server where your database is). Ask your hosting provider or it should be listed in your CP where you create your databases. Then fill in the name of your database and its passoword.
I think everything else is correct.
Hope this help

Please make bigger screenshots next time
You are missing a language file. Check if you have uploaded lang_main_cback_tracker.php where it is suposed to be. The correct path to the language file is written in the error message you have.

 Re: Instalation Problems
The Portuguese language pack you are trying to use is totally outdated and was written for an earlier version of Icy Phoenix.
It contains $lang files that are no longer used, and missing all of the new ones that are used with v.1.2.0.27b (The version you have installed)
It also appears to have never been finished anyway.
Maybe you would like to translate the latest English version to Portuguese? - - Maybe not.
To overcome the "Missing" files problem and to access your board - Simply upload ALL of the English lang files and overwrite the Potuguese ones.

And please take some time exploring your Icy Phoenix software, and search or read the docs where possible, before posting general questions about how to do something.
