what versione do you have installed of icy? i think the 1.0.9.9    because it happend me before upgrade  

I think you didn't replace/upload all the needed files.

Check you have these from latest packages:

• include/page_header.php
  
• templates/mg_themes/overall_header*.tpl
  
• templates/mg_themes/mg_themes.cfg
  
• templates/mg_themes/*.css
